Service Description

This option is for established Zolara Health patients who are due for a routine weight management follow-up or medication refill. Your follow-up is completed asynchronously, meaning you do not need to schedule a live phone or video appointment. You’ll complete a detailed medical questionnaire about your current medication and dose, weight progress, appetite and cravings, side effects, nutrition, hydration, activity level, and any changes to your medications or medical history. Your provider will personally review your responses, evaluate your progress and treatment plan, and determine whether a medication refill or dose adjustment is clinically appropriate. If additional information is needed, your provider may contact you through secure messaging or recommend a phone or video visit. Refills and dose increases are not automatic and are provided only after clinical review. Please complete all required follow-up questions thoroughly and accurately to avoid delays in your treatment plan. If you are experiencing significant or worsening side effects, a new medical condition, pregnancy, hospitalization, or another major change in your health, please contact Zolara Health before submitting a routine refill request, as a live follow-up or additional evaluation may be needed.
